Required Health Records

Students will not be able to attend school until we have all required forms on file, including:

  • Updated parent profile in My BackPack
  • Health Inquiry Form
  • Emergency contact information for at least two individuals
  • Current immunization records from your doctor

Forms are found in Magnus and My BackPack. These records will ensure that we have all of the information necessary to expeditiously handle any emergencies involving your child. Please be sure that your contact information on My BackPack is updated at all times—especially with phone numbers where parents can be reached.

The due date for these forms is July 15 for the following school year. Forms must be on file before your child starts school in August.

Magnus

Magnus is our student medical information database accessible through our website. With this online system, parents will have continuous access to their child’s health record and the ability to make updates when needed.

We ask that you enter the health information and other forms required within your Magnus account for each child attending Post Oak no later than the beginning of August for the new school year. 

Timely submission of health information and other forms is vital so that preparations can be made prior to the first day of school. Starting on April 1, you will receive weekly reminders as long as there are outstanding requirements in your account. Please reserve 20 to 30 minutes per child to complete the forms in Magnus. Parents will not have to repeat this work the following year; simply update the record as needed.

Accessing your Magnus account

Use this link or visit the Parent Portal landing page or My BackPack to access Magnus. Use your My BackPack credentials to log in to your Magnus account.

Immunization Compliance

The Texas Department of Health has ruled that students must be current with immunizations in order to attend school unless an exemption has been filed with the school in accordance with Texas Education Code. Parents must submit their child’s most recent immunization records to Magnus. Download the Texas Minimum State Vaccine Requirements for Students Grades K–12 and Child-Care Centers from the Texas Department of State Health Services website. Exceptions for Immunization Compliance

Any student enrolled for the first time in The Post Oak School after March 24, 2020, must fulfill the vaccine requirements under Texas law or submit a documented medical waiver of those vaccine requirements. All medical waivers must state medical diagnosis and will be reviewed and considered by the school nurse. Non-medical waivers of the vaccine requirements will not be accepted for enrollment to The Post Oak School.

Provisional Admission

A student may be provisionally admitted or enrolled if the student has begun the required immunizations. The student must have an immunization record that indicates the student has received at least one dose of each age-appropriate vaccine specified in the regulations.

Completion of Vaccinations

To remain enrolled, the student must continue to receive the necessary immunizations as rapidly as medically feasible. The student must complete the required subsequent doses in each vaccine series on schedule and as rapidly as is medically feasible and provide acceptable evidence of vaccination to the school nurse. 

Review of Status

The school nurse will review the immunization status of a provisionally enrolled student every 30 days to ensure continued compliance in completing the required doses of vaccination. If, at the end of the 30-day period, the student has not received a subsequent dose of vaccine, the student is not in compliance and will not be allowed to return to school until the required dose is administered.
